How entries get approved

  1. A monitor detects a change and stages it as pending.
  2. A human verifies the source and the before/after values.
  3. Approval publishes the entry — with its source, effective date and reviewer, appended to this ledger.
  4. Corrections are logged, never silently edited. See the corrections policy.
